DOVER-FOXCROFT - Mayo Regional Hospital successfully weathered what was a difficult financial year for rural and community hospitals in 1999. Drastic reductions in Medicare spending due to the federal Balanced Budget Act, coupled with continued managed care pressures from private insurance plans and HMOs, created budgeting challenges. Mayo met those challenges and recorded some noteworthy accomplishments. In two initiatives that helped position the hospital to meet community health needs assessment to identify healthcare priorities in the Penquis region, and finished a facility master plan that developed a comprehensive strategy for the hospital's future space needs. As a result of the needs assessment, Mayo and others are pursuing healthcare initiatives aimed at improving the health status of the region. The facility master plan will lead to a $6.3 million construction/renovation project that should be undertaken in 2001 to expand space for the Emergency Department and ambulatory surgery services, modernize obstetrical service space and construct a new Mayo Resource Center office and educational building. Mayo earned a three-year reaccreditation from the Joint Commission on the Accreditation of Healthcare Organizations. A three-day, on-site survey in June resulted in Mayo receiving a score of 90 from the Joint Commission. Technological improvements at Mayo included a new, fixed-base CT scanner with spiral capabilities, and the addition of mobile Magnetic Resonance Imaging services. In addition, the hospital completed a major project over the summer with the implementation of a new computer information system that provided support in managing patient and financial information.That computer upgrade played a key role in Mayo's ability to resolve Year 2000 compliance issues, and led to a trouble-free transition. Mayo Regional Administrative Services Corporation, a hospital subsidiary that owns and operates physician practices in Dexter, Dover-Foxcroft, Guilford and Milo, also enjoyed a productive 1999. MRASC and the town of Guilford jointly completed a construction project that doubled space in the town-owned medical building used by Guilford Medical Associates. The corporation in August opened a new primary care practice in Milo, staffed by a family practice physician and a physician assistant. Physicians added to Mayo's medical staff in 1999 were Kathleen Thibault, D.O. at Milo Family Practice, and general surgeon Peter Buerger, M.D., at Mayo Surgical Associates. Community health initiatives in 1999 included Mayo's enrollment in the Maine Breast and Cervical Health Program, and two school-based programs: Mayo worked with the Harmony School Department to open Maine's first K-8 school-based health clinic, and collaborated with SAD 68 and Foxcroft Academy to revive their student health clinics. The hospital also became the local sponsor for AmericasDoctor.com, a free, full-service, medical/health information Internet site. Mayo finished 1999 with a consolidated net income of $304,785, as outpatient services represented 54% of gross revenues. Average daily census was 15.4 patients. During the year the hospital invested $1.9 million in capital equipment designed to bolster clinical capabilities. |

A subsidiary of the hospital, the Mayo Regional Administrative Services Corporation, operates medical offices in Dover-Foxcroft, Dexter, Guilford and Milo serving over 10,000 people. The hospital is licensed by the state of Maine and in 1999 was reaccredited by the Joint Commission on Accreditation of Healthcare Organizations. The active medical staff includes 17 physicians board-certifies in the following specialities: anesthesiology, family practice, internal medicine, general surgery, ophthalmology, orthopaedic surgery, and radiology. There is a multispecialty consultant and associate staff of physicians and allied health specialists. Inpatient services are acute medical/surgical care, intensive care, surgical services and obstetrics. Outpatient services include 24-hour emergency care, ambulance, radiology, cardiopulmonary, day surgery, diabetes clinic, diet counseling, Heartwise cardiac rehabilitation, Imaging Center for Women, WorkWISE occupational health, oncology clinic, physical therapy, laboratory, prenatal clinic, substance abuse treatment, Lifeline and Pine Tree Hospice. The hospital's Imaging Center for Women provided free mammograms during National Breast Cancer Awareness Month to 15 women aver age 35 who had never had a mammogram and had no insurance. Mayo joined the Maine Breast and Cervical Health Program to provide mammography screening services for financially disadvantaged Maine women. The hospital extended its participation to include free breast biopsies for eligible women. Mayo's exhibit at the annual Chamber of Commerce business expo at Guilford in April provided 133 free blood pressure checks and free cholesterol tests, and dozens of free vision screening and lung capacity tests. Mayo and SAD 46 Adult Education sponsored a series of five, free community health forums at Dexter Regional High School. The hospital's Heartwise program provided 420 blood pressure screenings at free community clinics located in Dover-Foxcroft, Guilford and Sangerville. Heartwise community education efforts during American Heart Month included labelling heart healthy foods at local supermarkets. Mayo employees donated 27 pints to a Red Cross blood drive. |

Gross revenue for fiscal year 1999, Medicaid was billed for 9% and Medicare accounted for 52% of the services offered by the hospital last year. Payment is less than cost for many of those services. The unreimbursed cost providing this care causes other payers to be charged more to make up for those governmental shortfalls. Commerical insurances such as Blue Cross/Blue Shield were billed for 29%, 8% were people billed who had no insurance and 2% was other insurances such as worker's compensation.
